About Us

A Geriatric Care Manager (GCM) is a health & human services specialist who provides assessments, careplans, coaching and education for families who are concerned and caring for older relatives. Geriatric Care Managers can provide coaching regarding challenges that come with aging: including cognitive decline and Alzheimer's dementia, other common medical conditions, how to navigate day to day issues, and family communication and reasonable expectations. GCM's are also instrumental in managing crisis situations, including ER, hospitalizations and rehab stays. GCM's can advise families on avoiding costly but common mistakes. Additionally, they can assess the living situation and make recommendations and implement care plans to help the senior and families meet their goals.
